Pharmacy Change Requests

In the event medication(s) are requested to be sent to an alternate pharmacy, for ANY reason outside of the pharmacy being permanently closed, a $50 pharmacy change fee will be accrued. This applies to pharmacy change request subsequent to your scheduled appointment.
** Pharmacy change will be granted on a case by case basis and is at the discretion of the prescribing provider.
