Insurance (Co-payments/Deductibles/Billing)

You are expected to pay your deductibles and co-payments at the time of services. If we have not received payment from your insurance company, you will be expected to pay the balance in full. You are responsible for all charges. Please verify with the practice to see if your insurance is accepted. Please provide full insurance information and your insurance card upon your initial visit to determine eligibility of benefits. If necessary, please obtain authorization from your insurance carrier prior to your first visit. If your insurance plan requires pre-authorization for services, it is ultimately the Patient’s responsibility to obtain this authorization, prior to your appointment. If you fail to obtain authorization, any and all charges incurred and not reimbursed, will be your financial responsibility.

We reserve the right to bill the Provider’s standard fees, as modified from time to time and prorated to reflect actual time spent by the Provider, for care management, legal affairs, and phone consultations exceeding 10 minutes per week. Our time is valuable, and is best served providing high quality care to you while you are here in session. There is no charge for routine telephone calls to our administrative staff regarding scheduling, appointments, or billing.
